My athlete and I understand Westside Cubs Cheer Org will not tolerate any behavior unbecoming of a young lady and adult.

My athlete and I understand if ANY Westside Cub Cheer Coach or staff member is disrespected it will result in immediate removal without a refund.

My athlete and I understand if either of us harass the Westside Cub Cheer coaches or staff member it will result in immediate removal without a refund.

My athlete and I understand if the morale or integrity of the Westside Cubs Cheer program is comprimised by direct comments or action on our behalf it wil result in immediate removal without a refund.

My athlete and I understand if either of us harass the Westside Cub Cheer coaches or staff member it will result in immediate removal without a refund.

My athlete and I understand if the Attendance Policy is not maintained my athlete will not be able to participate in certain performances. (including but not limited to competitions.)

I certify that all of the information above is accurate to the best of my knowledge and have read, understand and agree to all the terms of the Westside Cubs Cheer registration packet including ALL fees and deadlines. I am also in agreement to understanding that this serves as a binding contract between me and the Westside Cubs Cheer Organization.
